Salary Range:     $36,000-$40,000 (salaried, exempt)

Schedule:  typically M-F days, but will be based on agency needs; must be flexible

Requirements:  Bachelor’s degree and three (3) years of experience providing services to individuals with intellectual/developmental disabilities and their families. Experience must be the equivalent of a year of full-time documented experience working with adults (21 or older) with intellectual/developmental disabilities.

 Job Duties:

  • Facilitate or participate in meetings and committees including support groups
  • Interview individuals and their families to determine goals
  • Identify safety, medical, financial
  • Create comprehensive plans
  • Research and initiate linkages to appropriate resources and services
  • Accompany clients to appointments
  • Secure emergency care interventions
  • Facilitate self-determination goals
  • Make contact via telephone, conduct in-home visits and monitor progress
  • Advocate for consumers and families with significant individuals and/or groups
  • Record services requested, actions taken, and all other significant events
  • Educate families, professionals and the community regarding case management services and individual/family needs to maximize the individual and family satisfaction
